Physician's patients are able to enjoy the comfort and security of the home setting and relearn the activities of daily living in their own environment. For the Primary Care Team Our Physicians work directly with the patient's primary and specialty physician and remain in constant communication during the recovery process through to discharge. Physician's Nurse Practitioners communicate with case managers and discharge planners on patient progress and outcomes.
